We realize that having the option to choose your own health care provider and facility is important and our VCU Health System ChoiceCARE and ChoiceEXTEND medical plans allow team members and their covered family members the freedom to choose where they seek their medical care. (Note: The ChoiceEXTEND plan is only offered to team members whose home address is outside the state of Virginia.)
With Sentara Health you get quality, affordable health coverage, and a large national provider network. The plan offers a three-tiered benefit structure.
- Tier 1: VCU Health system network of providers and facilities. This is the best benefit level offering the lowest out-of-pocket cost share.
- Tier 2: The Sentara PPO network of providers and facilities. This network is centralized for the state of Virginia and northern North Carolina. For coverage outside of this area, we contracted with PHCS/Multiplan for expanded coverage.
- Tier 3: No network participation. This is your highest cost of coverage and may result in significant out-of-pocket expense, unless emergency care is received.
